Thank you for selecting Assurance Wellness, LLC to provide your personalized rehabilitative care. For your first visit we want to make sure you’re comfortable and will have all of your concerns and questions addressed prior to the session. Prior to your session, you will receive an email to set up your account and complete paperwork on the PT Everywhere website and/or application. Please allow 15-30 minutes to complete this task.

Please wear comfortable, cool clothes and tennis shoes to therapy. Your visits will last approximately 60 minutes. Also, make sure you’ve eaten at least an hour prior to the session.

Your therapist will assess your progress throughout your course of rehabilitation. A customized home exercise program will be provided to you which may include instructions on exercises, posture, body mechanics, home/work ergonomics, nutrition, and how to manage pain to help you to reach your goals.
